Patient Questionnaire

Introduction

The Doctor may want to ask you some questions about existing health conditions, and similar questions that will aid the doctor in giving you the best treatment. If so, you will be asked to complete the questionnaire online. If the doctor doesn’t use this, a message will display if you click on the questionnaire menu option “Questionnaire does not exists.”

Selecting Questionnaire from the Main Menu will display the Patient Questionnaire page.

This page is used by the doctor’s office to ask the patient questions that will assist the doctors in treating you.

Some questions may be multiple choice, and some may have several choices that apply. Other questions may ask you to type your answer into a text area. This isn’t a test! It’s an easy way to provide information for the Doctor before your visit.

When you have completed the questionnaire, click the submit button at the bottom of the screen. Another screen will display with all you questions and answers listed. If everything is correct, click the submit button again. If you need to change something, click the modify button to return to the screen where you can change your answers.

When you confirm your answers by clicking submit a second time, you will see a messaged displayed “Your answers to the Questionnaire have been saved.” You’re all done! And thank you.
